Agudath Israel of America Halachic Medical Directive

Please Note: This form has only been approved for use in New York. Residents of other states should contact Agudath Israel of America 212-797-9000 for the appropriate form, or download your state’s form at JLaw.com, Halachic Forms.

In a dire emergency, it would be advisable to duplicate and use the New York form even for residents of another state. While the hospital might give the family a hard time with the form, as a legal matter at the very least, the executed document would serve as an indication of the wishes of the patient.

Rabbinical Council of America Halachic Health Care Proxy

Please Note: To further assist Torah-observant families, both Agudath Israel of America and the Rabbinic Council of America have partnered with the New York Legal Assistance Group (NYLAG, www.nylag.org) to store such documents in a secure, web-based registry, the U.S. Living Will Registry®, free of charge. This allows ease of access for any provider, at any time, anywhere in the world, should there be the need for a medical intervention and the hard copy is not available.

Zarit Screen Measure of Caregiver Burden

Gerontologic health scientific literature identifies a number of scales to measure caregiver burden. The Zarit Scale of Caregiver Burden or the Zarit Burden Interview is the most widely used instrument.  Originally designed and tested in 1980 containing 29 items, it was reduced to 22 questions. Subsequent adaptation of the scale made it particularly attractive. The research reported in The Gerontologist (2001, Vol 41, No. 5, 652-657) that a short 12-item version and 4-item screening version were found to correlate well with the full 22-item version.  The short and simple 4-item screen, proven to be valid and reliable for its designated use, is self-administered by the caregiver. A score of 8 indicates high burden, and intervention may be indicated. The screen has proven to be a helpful resource tool for caregivers and their families.

To be completed by caregiver.

Directory of Kosher Senior Centers

This listing is a work in progress. We welcome your comments (info@lorechyomim.org). Your experiences will help others to decide which centers they might wish to select. The directors also indicate that they would be happy to consider additional programming sessions, such as a men’s shiur, and would welcome those in the community who have the interest and expertise to offer such. In most instances, an attractive facility is there, in the heart of the community. It is an excellent resource for both educating and learning.

Kosher Adult Day Programs in New York State

Adult day programs are great. They offer more structured support for the participant than senior centers and needed respite opportunity for the caregiver. Moreover, they extend the amount of time an elder can remain at home rather than being admitted into a facility.
